Отчетность о проведении тестирования

Отчет заполняется старшим рабочей группы по проведению тестирования и подписывается всеми ее членами.

Отчет предоставляется в день проведения тестирования в структуры, уполномоченные для обобщения результатов анализа.

Форма отчета устанавливается в Приложении № 12 (на примере обследования учащихся на предмет злоупотребления алкоголем).
