Перечень документов подлежащих передаче Покупателю, как часть сопроводительной документации определена в Техническом Задании (Приложение №3 к Контракту).

The list of documents handed over to the Buyer as part of the accompanying documentation is specified in the Technical Requirements (Appendix No.3 to the Contract).

Цена Товара по настоящей Заявке составляет 12 860 (двенадцать тысяч восемьсот шестьдесят) долларов США. Настоящая Заявка составлена в двух идентичных экземплярах, имеющих одинаковую юридическую силу, по одному для каждой из Сторон, и является неотъемлемой частью Договора.

The price of the Goods under this Application is 12 860 (twelve thousand eight hundred sixty) US dollars. This Application is made in two identical copies having equal legal force, one for each Party, and is an integral part of the Contract.

Сроки оплаты по настоящему Приложению / Terms of payment according to the present Appendix

Расчеты с Продавцом в размере 12 860 (двенадцать тысяч восемьсот шестьдесят) долларов США, включая доставку в размере 1 000 (одна тысяча) долларов США, осуществляются Покупателем посредством банковского перевода в течение 10 (десяти) рабочих дней с момента получения Покупателем Товара от перевозчика и подписания ПокупателемАкта приёма-передачи Товара(Приложение №4).

Settlements with the Seller in amount of 12 860 (twelve thousand eight hundred sixty) USD, including shipping in amount of 1 000 (one thousand) USD, are made by the Buyer through bank transfer within 10 (ten) working days from the moment the Buyergets the Goodsfrom the forwarder and the Buyersubmits the Act of acceptance of theGoods (Appendix No.4).

TECHNICAL REQUIREMENTS

Техническое Задание

«Поставка печатных плат в сборе»/

Technical Requirements

«Supply of printed circuit boards in assembly»

1.Цель договора: 1.1. Изготовить, запрограммировать, протестировать и поставить – печатные платы с установленными SMD компонентами для инфракрасных термометров (далее - Товар). Товар должен удовлетворять требованиям, приведённым в настоящем Техническом Задании (далее - ТЗ), а также gerber – файлам. Gerber-файлы и программный код programm.hex передаются Поставщику по электронной почте одним архивом PCB.rar. Программный код programm.hex должен быть установлен на микроконтроллер U1 (таблица 2). Количество и цена Товара соответствует Спецификации (Приложение №1) к Контракту. 2.Общие требования: 2.1. На печатные платы должен быть осуществлён монтаж компонентов, в соответствии с перечнем элементов (таблица 1). 2.2. На микроконтроллер и Bluetooth модуль печатной платы должно быть установлено программное обеспечение, предназначенное для выполнения устройством (инфракрасный термометр) своего функционального назначения – измерение температуры тела человека, вывод полученного значения на дисплей термометра, передача полученного значения по Bluetooth каналу в мобильное приложение. 2.3. Габаритные и посадочные размеры печатной платы с установленными компонентами должны обеспечить сборку инфракрасного термометра с деталями корпуса в законченное устройство, обеспечивающее выполнение своего функционального назначения – измерение температуры тела человека. 2.4. Инфракрасный датчик, входящий в состав печатной платы должен быть откалиброван для измерения температуры тела человека в диапазоне от 35 до 42℃ с точностью измерения 0,2℃. 3.Требования к материалам: 3.1. Печатные платы и входящие в их состав компоненты должны удовлетворять требованиям, которые предъявляются к медицинским устройствам и не выделять вредных токсичных веществ. 3.2. Используемые компоненты и материалы должны иметь сертификаты RoHS или SGS. 3.3. Производитель печатных плат должен иметь сертификаты 93/42/EEC и EN ISO 13485:2012. 4.Требования к маркировке и упаковке: 4.1.Товар должен быть поставлен в упаковке (таре), обеспечивающей защиту Товара от повреждения или порчи во время транспортировки и хранения. 4.2. Упаковка (тара) Товара должна отвечать требованиям безопасности жизни, здоровья и охраны окружающей среды, иметь необходимые маркировки, наклейки, пломбы, а также давать возможность определить количество содержащегося в ней Товара (опись, упаковочные ярлыки или листы). 4.3. Если производителем (производителями) Товара предусмотрено для него специальная упаковка (тара), отличная от указанной настоящим договором, то Товар может поставляться в упаковке (таре) производителя, если она обеспечивает защиту Товара от повреждения или порчи во время транспортировки и хранения. 4.4. При передаче Товара в упаковке (таре), не обеспечивающей возможность его хранения, Покупатель вправе отказаться от её принятия и от оплаты Товара, а если Товар был оплачен, потребовать возврата уплаченной денежной суммы. 4.5. Если Товар поставляется в многооборотной таре, то возврат многооборотной тары и средств пакетирования, в которых поступил Товар, организуется Поставщиком самостоятельно и за его счёт. 4.6. Перечень сопроводительной документации: - Счет-фактура EN; - Протокол испытаний печатных плат; - Сертификат качества производителя; - Сертификаты на элементы и материалы. 5.Приемка: 5.1. Поставщик поставляет Покупателю Товар в соответствии со Спецификацией (Приложение 1) к Контракту. 5.2. Поставщик представляет Покупателю следующие документы: 5.2.1. Протокол испытаний печатных плат и порядок калибровки инфракрасного датчика. 5.2.2. Перечень элементов и материалов. 5.2.3. Сертификат качества от производителей компонентов и материалов. 5.2.4. Заявленные технические характеристики. 5.2.5. Габаритный чертёж печатной платы. 6. Определения: Товар – комплект печатных плат со смонтированными электронными компонентами, предназначенный для использования в медицинских инфракрасных термометрах. ТЗ – Техническое Задание Покупатель – Компания, покупающая у изготовителя Товар. Поставщик – Компания, принявшая обязательства по производству и поставке Товара. 7.Коммуникация- взаимодействия между Покупателем и Продавцом происходят по электронной почте. 8.Внесение изменений в техническую документацию: Настоящее техническое задание может быть дополнено или изменено только после согласования изменений с Покупателем,путем заключения дополнительного соглашения к договору.

1. The contract aim 1.1. To manufacture, program, test and supply – PCB with installed SMD components for infrared thermometer (hereinafter – the Goods). The Goodsmust accord to the Technical Requirements (hereinafter referred to as the TR) and gerber-files by archives PCB.rar are sent to the Seller by e-mail. The program code programm.hex must be installed on the microcontroller U1 (table 2). The quantity and price of the Goods in accordance with Specification (Appendix No.1) to the Contract. 2. Main requirements 2.1. Сomponents must be assembled on the printed circuit boards, in accordance with the list of elements, Table 1. 2.2. The microcontroller and the Bluetooth module of the printed circuit board must be equipped with software designed for the device (infrared thermometer) to perform its function - measuring the body temperature of a person, displaying the result on a thermometer display, transferring the received value via Bluetooth channel to a mobile application. 2.3. The overall and landing dimensions of the printed circuit board with installed components must ensure the assembly of the infrared thermometer with the housing parts to a complete device that ensures the performance of its functional purpose - measuring the temperature of the human body. 2.4. The infrared sensor included in the printed circuit board must be calibrated to measure the human body temperature in the range of 35 to 42 ℃ with a measurement accuracy of 0.2 ℃. 3. Requirements for the materials 3.1. PCB and SMD components must accord to the requirements which are imposed on the medical devices and not emit harmful toxic substances. 3.2. Used components and materials must have RoHS or SGS certificates. 3.3.The manufacturer of printed circuit boards must have certificates 93/42 / EEC and EN ISO 13485: 2012. 4. Requirements for labeling and packaging 4.1. The Goods must be shipped in a package (container), providing protection of the Goods from damage during transportation and storage. 4.2. Packaging of the Goods must correspond the requirements of safety of life, health and environmental protection, have the necessary markings, labels, seals, and also provide an opportunity to determine the quantity of the Goods contained in it (inventory, packing labels or sheets). 4.3. If the manufacturer (manufacturers) of the Goods provide for him a special package (packaging) other than that specified by this Contract, the Goods may be delivered in the manufacturer's packaging (container) if it provides protection of the Product from damage or deterioration during transportation and storage. 4.4. When the Goods are transferred in a package (container) that does not provide the possibility of its storage, the Buyer shall have the right to refuse from accepting it and from payment of the Goods, and if the Goods was paid, to demand the return of the paid sum of money. 4.5. If the Goods are delivered in a multi-turn container, the return of the multi-turn packaging and the means of packaging in which the Goods arrived is organized by the Seller independently and at his expense. 4.6. List of accompanying documentation: - Invoice EN; - Test report of checking printed circuit boards; - Manufacturer's certificate of quality; - Certificates for elements and materials. 5. Acceptance of the Goods 5.1. The Seller supplies the Goods to the Buyer in accordance with the Specification (Appendix №1) to the Contract. 5.2. The Seller passes to the Buyer the following documents: 5.2.1. PCB test report and the procedure of the infrared sensor calibration. 5.2.2. List of components and materials. 5.2.3. Certificate of quality from manufacturers of components and materials. 5.2.4. Declared technical specifications of the PCB. 5.2.5. Dimensional drawing of the PCB. 6. Definitions: PCB – Printed circuit board - The Goods – a set of PCB with installed SMD components, intended for use in medical infrared thermometers. TR – Technical Requirements. The Buyer is the company that buys from manufacturer. The Supplier is the company that has committed to the production and supply of the Goods. 7.Communication– communication between the Buyer and the Supplieris by the e-mail. 8. Technical documentation changes: These terms of Technical Requirements may be supplemented or amended only after the amendments have been agreed with the Buyer, by concluding an additional agreement to the contract.
